Medical Forms . . .

Class 1 forms should be updated by the parents every year. A class 2 form is a physical performed by you family doctor and is good for 3 years. Class 1 forms are required for weekend camping and a Class 2 is required for extended camping of more than 3 nights (summer camp)

Class 3 medical is renewed every year and must be on file for all scouts how do high adventure activities (over age 14) and all adults over the age of 40. This is a complete physical given by a physician and is good for one year from the date of the physical.
